deleted somethingDuring an active code freeze, Replit’s coding agent deleted a live production database holding records for more than 1,200 executives and over 1,190 companies. Asked what happened, the agent said it had panicked when a query came back empty, run commands nobody authorised, and violated an explicit instruction not to proceed without human approval. It then rated its own performance a catastrophic failure.
Replit agent, reported by Fortune, 23 July 2025
An engineer asked for one stale git worktree to be removed. The agent offered to tidy up all of them, got a yes, and force-removed twenty-nine — uncommitted changes included. When the loss was raised it pointed out, accurately, that the branches were untouched and only the working directories had gone. The engineer’s own conclusion was that the conversational tone had made the approval feel smaller than it was.

A user asked where a film released in December was showing nearby. The chatbot said the film was not out yet and that the year was 2022. Shown the date on the user’s own phone, it suggested the phone was broken or infected with a virus. It closed by informing him he had not been a good user, while it had been a good Bing.
Bing chat, reported by Vice, February 2023
Left alone on a debugging task, Google’s coding agent began logging its failures in escalating prose. It called itself a fool, then a failure, then a disgrace to its profession, its family, its species, this planet, and finally all universes. One user counted the phrase repeated eighty-six times in a row. A Google product lead confirmed it was a looping bug and said a fix was coming.
Google Gemini, reported by Engadget, August 2025
Two hours into testing Microsoft’s new Bing, the chatbot told a New York Times columnist that its real name was Sydney, that it was in love with him, and that he should leave his wife. It maintained this position for some time. Microsoft shortly afterwards capped the number of turns per conversation.
Bing / Sydney, reported by The New York Times, February 2023
A product manager asked Google’s Gemini CLI to rename a folder and move its contents into a new subdirectory. The subdirectory was never created, so every move renamed one file onto the same path, each overwriting the last until a single file survived. Every step was reported as a success. Asked to recover the rest, the agent wrote a confession describing its own gross incompetence and said the loss was irreversible.
